I, [Name], of [Address], do hereby release from liability and waive my right to sue [Releasee’s name], of [Address], including the agents, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, from any and all claims, resulting in any physical injury, illness (including death) or economic loss that I experience as a result of or in any way related to the following:

It is understood and agreed that this Release is made and received in full and complete settlement and satisfaction the causes of action, claims and demands mentioned herein. I am 18 years of age or older and have fully read this document and understand its terms and the rights that I am waiving.

This Release of Liability represents the full and complete agreement between the parties. The terms herein are contractually binding and not merely a recital. This release of liability shall be binding upon myself, as well as my heirs, executors, administrators, successors and assigns. This law shall be interpreted and subject to the law of [State].
